Just bone headed. You have a job that requires concentration for what, 3 hours a day, when you play. You forget how many outs there are then in your haste, you make a hurried throw and miss the cutoff man. That can’t happen. Pallante pitching his butt off and you can’t back up his effort.
Yes , it was a mental error. Yes, the games last about 3 hours for each one. We all agree it should not happen but all workers make a mental error sometime. In all my many work years I never came across a worker (player) who didn’t screw up once in a blue moon.
Anyone who says they never had a mental lapse is either lying, in denial about making a mistake or never did the job many hours.
Don’t make excuses for him, that’s just aiding his mistake. I’ll bet any job you ever had wasn’t a 3 hour job a few days a week with a 6 month vacation every year and if you made a boneheaded decision/mistake, there were probably consequences for those mistakes.
Sure it’s a high profile position but with that comes accountability. How many people paid to come and watch you do your job when you worked. I also don’t remember a time when I made a blatant mistake that there wasn’t some kind of accountability involved.
Time for me to turn the game off. If these guys are too lazy or gutless to hit .1000 and never make an error, it’s not worth watching.

How is it a mistake to try and stretch a 2 out hit that bounced off the wall into a double? I send him 100 times out of 100 there with 2 outs.

I did comment that he needs to learn how to slide better.
...Noot's error cost the tying run in the the top of the 4th inning... Plenty of time to get that run back... A more concerning problem is that the Cards are 1-for-9 with RISP after 6 innings...
